Recommended Adult Dose of Cabergoline

1. Initial Dose

0.25 mg of cabergoline taken orally twice weekly.

2. Maximum Dose

1 mg of cabergoline taken orally twice weekly.

3. Dose Titration

Based on the patient's serum prolactin level, increase the dose by 0.25 mg per twice-weekly administration, at intervals of at least 4 weeks.

Dose Adjustment Considerations for Cabergoline

1. Patients should be maintained on the lowest dosage that provides optimal effect.

2. After serum prolactin levels have normalized and been maintained for 6 months, cabergoline may be discontinued and prolactin levels monitored to assess the need for reinitiating treatment.

Dose Adjustments for Special Populations on Cabergoline

1. Renal Impairment: Dose adjustment is not required.

2. Hepatic Impairment: Use with caution.

3. Geriatric Patients: It is recommended to start at the lower end of the dosing range and adjust cautiously.

Contraindicated Populations for Cabergoline

1. Safety and effectiveness in patients under 18 years of age have not been established.

2. Contraindicated during pregnancy and lactation; contraindicated in patients with severe heart disease or history of psychiatric disorders (see Warnings section for details).

1. Periodic monitoring for fibrotic complications (e.g., erythrocyte sedimentation rate, chest X-ray, serum creatinine).

2. Cabergoline is not recommended for the purpose of postpartum lactation suppression, as it may increase the risk of hypertension, stroke, and seizures.

3. The durability of cabergoline's effect beyond 24 months has not been established.

Patient Guidance for Cabergoline

1. Seek immediate medical attention if experiencing difficulty breathing, persistent cough, orthopnea, or swelling of the extremities.

2. Avoid driving or operating machinery while taking cabergoline until the drug's effect on psychomotor function is known.

3. Be alert for symptoms of sudden onset of sleep, compulsive behaviors, or unusual impulses, and report them to the doctor promptly.

4. Consult the doctor regarding pregnancy planning, pregnancy, or breastfeeding.
